PRIVACY NOTICE
The National Institutes of Health (NIH) National Institute of Allergy and Infectious Diseases (NIAID) is committed to protecting your privacy. We value the trust and confidence you place in us. This Privacy Notice explains the collection and use of personal information about you when you request an account to access this system. To access this system, you need to provide your name and e-mail address in order to create your account. Please note that by requesting an account and accessing this system, you are accepting the practices described in this Privacy Notice and consent to our collection, storage, and use of personal information about you as described in this Notice.
PURPOSES AND USE OF INFORMATION WE COLLECT FROM YOU
The primary use of your name and e-mail address is to create your account to access the system, and for occasional communications from system administrators about system status. Your e-mail address may also be present in system logs.
In addition, when you visit a Web-based system, certain information about
your visit can be collected. This system automatically collects and temporarily
stores the following type of information about your system access:
- Internet address and domain from which you accessed the system
- Information about the operating system and browser used
- Date and time of access
- Pages accessed
- Internet address of the Web site that connected you to the system
PARTICIPATION IS VOLUNTARY
Providing your name and e-mail address is voluntary. However, if you do not provide this information, you will not be granted access to the system as this information is necessary to create your account. If you have an account and no longer need access, you can request your account be deactivated by sending an email to NIAIDSTORMSupport@mail.nih.gov. Note your name and e-mail address may remain in the system as accounts are disabled, and not removed. Your e-mail address may also be present in system logs, even after your account is disabled.
YOUR RIGHT TO CORRECT YOUR PERSONAL INFORMATION
The system allows you to make corrections to your name; however, if you need to correct your e-mail address, you can contact NIAIDSTORMSupport@mail.nih.gov for assistance.
HOW PERSONAL INFORMATION IS PROTECTED
This system has implemented administrative, technical, and physical controls to protect your personal information that we collect. Roles in the system are used to control who has access to your personal information. Policies for the retention and destruction of personal information are in place. Data travels over secured NIH networks. Intrusion detection is provided by the NIH network at the perimeter and other points within the network. Backups of system data are performed on a regular basis. System components are in NIH data centers, which have appropriate physical controls to restrict access to servers.
You can contact NIAIDSTORMSupport@mail.nih.gov if you believe your personal information has been inappropriately obtained, used, or disclosed.
HOW NIAID USES COOKIES
When you visit any Web-based system, its server may generate a piece of text known as a "cookie" to place on your computer. The cookie allows the server to "remember" specific information about your visit while you are connected. The cookie makes it easier for you to use the dynamic features of Web pages. Cookies from this system only collect information about your browser's visit to the system; they do not collect personal information about you.
There are two types of cookies, single-session (temporary), and multi-session (persistent). Session cookies last only as long as your Web browser is open. Once you close your browser, the cookie disappears. Persistent cookies are stored on your computer for longer periods.
The Office of Management and Budget Memo M-10-22, Guidance for Online Use of Web Measurement and Customization Technologies, allows Federal agencies to use both session and persistent cookies. This system uses session cookies to facilitate your secure access to the system. The session cookies only contain information about your browser's visit to the system; they do not contain any personal information about you.
HOW TO OPT OUT OR DISABLE COOKIES
If you do not wish to have session cookies placed on your computer, you can disable them in your Web browser. Instructions for disabling or opting out of cookies in the most popular browsers are located at https://www.usa.gov/optout-instructions. If you opt out of cookies, you may experience issues accessing this system. Please note that by following the instructions to opt-out of cookies, you will disable cookies from all sources, not just those from this system.

REVISIONS TO THIS NOTICE
We may revise this Notice at any time by posting an amended Notice on the system's login page. Any changes that we make to this Notice will be effective immediately upon posting. Please check this page periodically for changes to the Notice.
